Service Description

Advanced directives are your way of legally stating how you would like your end-of-life care handled, in advanced, in a legal document format for others to know and follow. This is a way to tell your wishes for your final moments to family, friends, and health care professionals and to avoid confusion later. This can include do not resuscitate (DNR), Physician Orders for Life-Sustaining Treatment (POLST), organ donation, and more.

Cancellation Policy

To cancel or reschedule without penalty, please notify us at least 48 hours prior to your appointment. A $50 non-returnable fee will be charged for any cancelations made less than 48 hours prior to the appointment. Only one reschedule will be permitted per client given you notify us at least 48 hours prior to the appointment. A $15 non-returnable rescheduling fee will be charged for any rescheduling unless you have contacted us 48 hours or more prior to the time of your appointment.
